This bag is new but I listed it as used because it was not in the package. It comes with shoulder straps. The fastening system on the back will hold it on your shoulders or you can carry it with one strap as a \"over the shoulder\" pack. Very nice piece of equipment. It would be great if you are planning to fix your own medical pack and put your own specific First Aid items in it. Great for campers, hikers, rescue, law enforcement or EMS.
