I rewired this to get rid of a frayed place that was going through the lamp. Bulb not included. recommend florescent or LED bulbs, the long incandescent bulbs these normally take put out too much heat, and a small twisty florescent gives out just as much light for less electricity. Other than rewiring and putting it back together, I did nothing else. it still has it\'s dings and flaws on the finish that it had when I got it. While I don\'t know where or when it was made, the UL sticker makes me think it was US made. I sent a pic of the sticker to UL labs, they might be able to help me identify it further, but as yet, I haven\'t heard back. Contact me about shipping, we never know what the calculator will do. I think a medium flat rate box at 15 dollars will work best for most places, and leave room in the box for other small items, hint hint.
